Other Keep blowing ADA Compression O ring!!

I'm having an o ring issue, i just blew my second compression oring on an ADA head in only 2 Hrs. Im debating a faulty install, but then again my cylinder sleeve is flush all the way around except for a 1-1/2" circumference spot that is actually raised almost the thickness of a piece of computer paper. What can be done in this situation? Should i even be concerned or just throw a third set of orings in? Can the tops of cylinders be milled? I understand how cylinders sleeves can drop, but can they raise? If so how could it raise in only one side and not the other, i feel like it was raised to begin with.

Its most likley you sleeve has dropped this exposes the o-ring to the flame front. Is it always blowing the in the same spot ??
 
i have checked the for warping, if the sleeves dropped wouldn't it be noticeable? I have never seen one drop before but i have heard of it. It has blown in the same spot. I didn't notice any chattering of the domes, but i will look again. Also, how necessary is it to completely change out girdled head bolts i have never changed mine and i've had my head off probably 5 times.
 
ADA heads do not last forever. eventually the head shell will not tighten down enough to sandwich the dome in there and O-rings will blow, due to clearance between the dome and sleeve. if the dome spins, trying to remove the plugs, its worn out. i have a couple worn ADA heads and i'm going to have 5 or 10 thousandths milled off the shell to tighten the domes up again.

I would doubt its the head they do wear out in time like all heads do but I cant imagine it id that just yet.
The way you have described the cylinder makes it sound like you have an issue with a non flat deck. personally I would getting the cylinder skimmed
