First time for me, the outer sleeve was wedged in a groove in the coupler. Had to grab it in my vice and hammer it apart.

yup, seen it before, from what I gather there were 2 designs, one had a thin sleeve like pictured, the other had a much thicker collar
Yeah, I looked at a few other cranks/engines I had around here and they had the thick collar (all 61x and 6m6). This came out of a 62t 2006 SJ, the SBT crank I am replacing it with has this thin sleeve as well.
I would have been upset if it wasn't a wasted crank.

I blew 2 DASA topends before this issues was found to be the root cause. Evidently it does not happen much but something I always check now. What crank is that? Mine happened on a 10mm crankworks

Does the coupler have a groove in it that just happens to be exactly in the right spot and the right size or did the sleeve somehow cut it?

That is a thin hardened wear ring. They slip onto the crank and get held firm by the soft aluminum coupler,. Sometimes they will stick to the coupler or just slide off. I clean them and reinstall with a drop of loc-tite.

This one ended up a couple/few millimeters inside of the end of the coupler.
