Super Jet throttle lever causing runaway ski

Hey guys, at the end of one of my last rides I did a nose stab, came up and my ski took off. I had one hand on the bars, pulled my self up and ran back to the launch and pulled my lanyard. What happened was the last 5 threads behind the adjusting nut (inside the throttle lever, you can see the extra threads through the slot on that nipple) had pulled out away from the lever and got stuck on the outter edge of that nipple thing. seems like it would give slack on the throttle cable but it reved the motor up almost 3/4 of the way until I pushed the threads back in the lever, and the nut was flush with the lever.
So, I guess I am wondering if I am missing a part, causing this to happen, or if there is a good way to make sure this will not happen again.

when the cable pulled out, it was making the patch from the lever to the carburetor longer, this is the same as pulling the finger throttle. You said you pushed the threads back in? that is an adapter that is suppose to screw onto your cable and then screw into the throttle lever........ It should not pull out or push in at all. If so, you need a new adapter or throttle assembly.

Looks like your adjustment is quite extended on the bar side. Try adjusting the carb side a little bit to allow more threads to grab on the finger throttle side
 
Here is the throttle adapter, the threads on the inside where the cable screws into are stripped smooth. Anyone know if you can get a new adapter like this?
