PWD form 143 and receipts showing materials used to build it. Call and schedule a time. Only certain offices do the inspection and you have to set an appointment. Have the PWD 143 filled out before you get there. They will have you fill out a game warden inspection affidavit. The game warden will go poke around your ski and might ask some questions. Total fees come out to $84.

They ask some questions that seem to be feel-er questions to see if you are being truthful. After chatting with the office ladies and the warden, they said that they very rarely see a true homemade vessel. Most of the time someone takes a boat, does some work to it, and then tries to register it as homemade to get a title. Apparently that's the reason for the questions.

There's a spot for not assigned by manufacturer. You could probably use the affidavit of fact stating you purchased it and show your receipt from tigercraft.

Hood after new clear coat and wet sanding/polish. Sunshield glosses out amazingly.
